By submitting this form, I acknowledge that my participation in SALP-sponsored activities is a voluntary privilege and not a right. I fully acknowledge the risks and hazards involved and agree to assume all risk of loss, injury, death or property damage
that may occur as a result of my participation in the activity or activities. I have no medical condition that prevents my participation and have the physical capacity reasonably necessary to engage in the activities. However, in case of an emergency, accident
or illness, I give permission to be treated by medical professionals if necessary and agree to be responsible for any expenses incurred as a result thereof.
I acknowledge that I have not consumed any substances that may cause me to not understand the meaning of this agreement. In consideration of my being permitted in the activity, I release, indemnify, and hold Portland State University, its employees, directors,
trustees, officers, and agents (“PSU”) harmless from any and all liability, claims, causes of action, damages or demands against PSU for any loss, injury, death, or property damage that may result from my negligence and participation in future activities,
whether caused by the negligence or carelessness of PSU or otherwise. I agree to comply with all rules and conditions associated with participating in SALP activities. I understand that conduct not acceptable during SALP activities may result in my removal
from the activity. It is also my understanding that this Acknowledgement and Release shall be binding upon my spouse, family members, heirs, guardians, legal representatives, and assigns.
I further agree to save, hold harmless and indemnify PSU from any claims by the aforementioned parties arising out of my participation in SALP Activities.
I recognize and acknowledge that PSU makes no guarantees, warranties, representations, or other promises relative to SALP Activities, and assumes no liability or responsibility for injury or property damage that I may sustain as a result of my participation.
I further understand and agree that this Risk and Waiver of Liability form is intended to be as broad and inclusive as permitted by law. If any portion hereof is held invalid, it is agreed that the balance shall, continue in full force and legal effect.
By submitting this form, I hereby acknowledge that I am at least 18 years old, that I have carefully read this document in its entirety, understand it, and sign it voluntarily.
